View Medicare Accountable Care Organization

ACOs are groups of doctors, hospitals and other health care providers, who come together voluntarily to give coordinated high quality care to Medicare patients they serve.  Coordinated care helps ensure that patients, especially the chronically ill, get the right care at the right time, with the goal of avoiding unnecessary duplication of services and preventing medical errors. When an ACO succeeds in both delivering high-quality care and spending health care dollars more wisely, it will share in the savings it achieves for the Medicare Program.

  1. Select the Financials button on the home page.

  1. In the Select a Provider panel, click the Select button next to the applicable provider account.

  1. Select Medicare Accountable Care Organization (ACO) from the Financial Options drop-down menu

  1. Select the applicable demonstration code from the DemoCode drop-down menu.
  2. Select the Search button.

If the Part A provider selected is part of a Medicare ACO the following information will display.

If the provider selected is not part of the ACO the following message will display. “No ACO information found for the provider selected.”

  • Record Identifier
  • Model Participant Identifier – this field will be blank
  • Provider Tax Identification Number (TIN) – 9-digit numeric value
  • Provider National Provider Identifier (NPI) – 10-digit numeric value
  • Provider CCN – this field will be blank
  • Record Type
  • Record Type Label
  • Effective Start Date – MM/DD/YYYY, the system will default to display a descending sort on the ‘Effective Start Date’ field
  • Effective End Date – MM/DD/YYYY
  • Part A Percentage Rate
  • Create Date – MM/DD/YYYY
  • Record Status
  • Last Update Date – MM/DD/YYYY
  • Status
  • Type
  • Maintenance Operator
  • Maintenance Date – MM/DD/YYYY
  • CAP
  • Provider Reduction
