Submitting Electronic Medical Records via CD-ROM
Providers have expressed interest in the ability to submit medical records for review electronically. National Government Services is pleased to announce that we are able to accept medical records submitted on a CD-ROM. We prefer providers load the documents onto the CD-ROM using WinZip®.
Documents included the CD-ROM should be for the same NGS business department. Do not send records intended for the Medical Review Department on the same CD-ROM as records submitted for the Appeals Department.
It's important that you include a cover sheet with each CD-ROM. The cover sheet should include:
- the business department to receive the CD-ROM, i.e., Medical Review, Appeals, etc.,
- the specific claim(s) information, i.e., beneficiary name, Medicare number, dates of service, and claim number, for each claim on the CD-ROM,
- the reason records are being submitted and
- the password, if applicable, so we can open the files included on the CD-ROM.
Please download and use the Cover Sheet for Electronically Submitted Medical Records to submit with each CD-ROM. Please note that this form is available for your convenience but is not required.
