Selecting the Contractor Code

Select the contract in which the provider of service is enrolled in the Medicare Program in the Contract Code field.

Click the drop-down arrow to display the available contractor codes. 

Select the applicable contractor code, then click Next.

Note: Each contractor code references a specific LOB and jurisdiction; it’s important to be sure to select the appropriate National Government Services Contractor Code.
