Testosterone Pellet Dosage

The dosage approved by the US Food and Drug Administration for testosterone pellet replacement therapy in androgen-deficient males is 150 mg to 450 mg subcutaneously every 3-6 months. Effective for dates of service 9/15/2014 and forward, NGS will not allow payment for more than 450 mg (six 75 mg pellets) of testosterone unless, despite treatment with standard doses, there is information accompanying the claim or sent upon request sufficiently documenting that a higher than usual dose is needed for the beneficiary due to symptoms of androgen deficiency accompanied by a sub-therapeutic testosterone blood level.
