Medicare Part B General Billing

Claims for drugs and biologicals are mandatory assignment. Claims submitted as nonassigned will be converted to adjudicate as assigned.

When billing drugs or biologicals, the drug name, dosage and invoice price must be entered into the notes section of the claim.

• The notes section is Item 19 of the CMS-1500 paper claim form
• The Medicare Part B CMS-1500 Crosswalk for 5010 Electronic Claims may be used to locate the proper location to report the drug name, dosage, and invoice price on the 837P, Medicare Part B electronic claim form

Example of how the information should appear in the notes section of the claim:

• DRUG: LIORSEAL
• DOSAGE: 80,000 MCG
• INVOICE PRICE: $2,376.37
