IMPORTANT: Action Required for SNF Revalidation and Attachment, Due Date is now 1/1/2026

This message notifies SNFs that the 8/1/2025, due date for submission of their Form CMS-855A revalidation application or, as applicable, SNF Attachment has been pushed back to 1/1/2026.

How to Revalidate

Revalidate Medicare enrollment information either electronically by using the internet based Provider Enrollment, Chain and Ownership System (PECOS) or by submitting the appropriate CMS Part A paper application. Revalidate by the due date to avoid deactivation of your Medicare billing privileges and/or lapse in coverage.

Don’t Forget To

• Pay the application fee.
• Have an authorized/delegated official of an organization or the individual provider sign and date the CMS form(s), include signatures of newly added authorized/delegated officials.

Already submitted your application and SNF Attachment!

• Respond timely to any follow-up requests for additional information and/or documentation.
