IMPORTANT: Action Required for SNF Due Date of 8/1/2025
For those SNFs that have not yet submitted their Form CMS-855A revalidation application or, as applicable, SNF Attachment, this is an important reminder that you must do so by 8/1/2025. Failure to do so could result in the deactivation or stay of your Medicare enrollment.

How to Revalidate…
Revalidate Medicare enrollment information either electronically by using the internet based Provider Enrollment, Chain and Ownership System (PECOS) or by submitting the appropriate CMS Part A paper application. Revalidate by the due date to avoid deactivation of your Medicare billing privileges and/or lapse in coverage.
Don’t Forget to…
- Pay the application fee.
- Have an authorized/delegated official of an organization or the individual provider sign and date the CMS form(s), include signatures of newly added authorized/delegated officials.
