Missing/Incomplete/Invalid Patient Identifier Remark Code N382

If you receive a denial on your remittance with remark code ‘N382’ Missing/Incomplete/Invalid Patient Identifier, please use the IVR or NGSConnex self-service option to verify the MBI number used on your claim is correct/active. If the MBI is termed, you must contact the patient, please do not call the Provider Customer Service team as they are not able to assist with this.

To verify the MBI in NGSConnex, select the Beneficiary Eligibility Information Panel under Eligibility Lookup. The Beneficiary Information section will provide the following:

  • Medicare Number
  • Last Name
  • First Name
  • MBI Term Date, if applicable – If a term date displays, you must contact the beneficiary to obtain the new MBI or use the MBI Lookup Tool, to search for the new MBI 
  • Date of Birth
  • Date of Death, if applicable
  • Sex
  • Address, City, State and Zip

If you use the IVR to verify Eligibility and the MBI is termed, you’ll be instructed to contact the patient or use the MBI Lookup Tool in NGSConnex to verify the correct/active MBI.
