Notification of Upcoming Due Date for Skilled Nursing Facility Ownership Disclosure Initiative

For those SNFs that have not yet submitted their Form CMS-855A revalidation application or, as applicable, SNF Attachment, this is an important reminder that you must do so by 1/1/2026. Failure to do so could result in the stay or deactivation of your Medicare enrollment.
