On 2/22/2021, NGS Part A MR Department initiated a service specific post-payment review of randomly selected claims billed for the following services mentioned in JK for Part A providers in the states of New York, Connecticut, Massachusetts, Maine, New Hampshire, Vermont and Rhode Island.

  • Reason Code 55H1M: The documentation indicates the general inpatient level of care was not reasonable and necessary. Therefore, payment will be adjusted to the routine home care rate.
    • 188 claims denied
    • Examples: The assessments indicate that the beneficiary is without pain, anxiety or respiratory distress and using PRN medications infrequently; the documentation did not support the intensity of care directed towards pain control or symptom management that could not feasibly be provided in any other setting.
  • Reason Code 55H1R: The notice of election is invalid because it doesn’t meet statutory/regulatory requirements.
    • 34 claims denied
    • Examples: The notice of election was not filed timely and the notice of election did not contain the waiver of traditional Medicare benefits.
  • Reason Code 55H1Y: The physician narrative statement was not present or was not valid.
    • 12 claims denied
    • Examples: The narrative statement did not include information to support the rationale for a six-month or less prognosis. Reference to an interdisciplinary team meeting is not an acceptable substitute for a signed physician narrative that presents clinical information about the patient’s terminal prognosis.
