Revalidation Request Emailed or Mailed
We will email or mail the revalidation notices to identified providers and suppliers currently enrolled in the Medicare Program. The revalidation notice will be sent two to three months prior to due date.
Each provider or supplier will receive their own revalidation notice, emailed or mailed in a yellow envelope.
- Emails will initially be sent to your correspondence email address and/or the contact person’s email address.
- If both emails are undeliverable, the revalidation notice will be mailed in a yellow envelope.
- Mailing will be sent to the special payments address and to the correspondence address.
- If both addresses are the same, a second notice will be mailed to a provider’s practice location address.
- If all three addresses are the same, only one notice will be mailed.
Important Notes
- Due date is stated on email or mailed notice
- Failure to submit by due date or not responding timely for additional information will result in your Medicare payments being held or deactivation of billing privileges.
- Revalidation application received more than seven months prior to the due date are considered unsolicited applications and will be returned.
