To reactivate your individual and/or group billing privileges, please follow instructions within the deactivation letter.

Instructions for Accessing PECOS

For first-time user, request a user ID and connection (if applicable) or log in with your user name and password.

Instructions for Reactivating in PECOS

• Login to PECOS
• My Associates
• View Enrollment (enrollment in question)
• Select Enrollment record (applicable for reactivations)
• Select More Options
• Select Reactivate
• Start Application/Confirm Reason
• Review and Update all Topics
• Resolve all Errors and Verify Warnings
• Begin Application Submission
• Submit and Sign the Application(s)

Note: If both the sole owner and the sole owners group are deactivated, a PECOS reactivate application is required for each.

Instruction to Reactivate Using Paper Application

To reactivate an individual provider (physician or nonphysician practitioner) (sole proprietor or sole owner):

• Download and complete CMS-855I  

Follow instructions in the hyperlinks below for applicable situations and mark “reactivate” in section 1 of the paper application:

• Individual (Group Member) Reassigning Benefits Only
• Individual Medicare Enrollment Private Practice ‒ (Sole Proprietor) via SSN
• Individual Medicare Enrollment Private Practice ‒ (Sole Proprietor) via EIN
• Individual Medicare Enrollment Sole Owner of a Corporation via Tax ID  
• Physician Assistant Medicare Enrollment

To reactivate a clinic/group/entity/supplier (type 2 NPI)

• Download and complete CMS-855B

Follow instructions in the link below for applicable situations and mark “reactivate” in section 1 of the paper application:

• New Provider Corporation or Group ‒ (Multiple Owners)
