If a CAH wants to become a Method II biller, email the request to A&R at ngsreimbursement@anthem.com.

- Doctor of Medicine (MD)
- Doctor of Osteopathy (DO)
- Doctor of Chiropractic (DC)
- Doctor of Dentistry (DMD) (DDS)
- Doctor of Optometry (OD)
- Doctor of Podiatry (DPM)
- Physician Assistant (PA)
- Clinical Nurse Specialist (CCNS)
- Nurse Practitioner (NP)
- Clinical Psychologist (CP)
- Certified Registered Nurse Anesthetist (CRNA)
- Registered Dietitian/Nutritional Professional (RD or NP)
- Marriage and Family Therapist (MFT)
- Mental Health Counselor (MHC)
Individual provider must have an active enrollment (CMS-855I) to reassign to a Part A Entity (e.g., critical access hospital – CAH II)
