Please Note: General inquiries related to Medicare coverage policies, including clinical issues, drugs, radiopharmaceuticals, local and national coverage determinations, billing and reimbursement must be directed to the Provider Contact Center. The PCC is responsible for assisting our provider community and are trained to answer your questions.  If the PCC requires additional information from other departments to answer your inquiry, it will coordinate that effort and respond back timely to you.

LCD Reconsiderations

Submit Reconsideration Requests to NGS.LCD.reconsideration@elevancehealth.com.

The LCD Reconsideration mailbox is intended only for the submission of LCD reconsideration requests specific to an active LCD, as outlined in Article A52842, Reconsideration Request Process.

Commenting on Proposed LCDs

When proposed LCDs are available, comments can be made by viewing the Proposed LCD on the Medicare Coverage Database and selecting the “Submit Public Comment” button or by selecting the “Contact for Comments on Proposed LCD” email link at the end of the LCD. Comments will only be considered if submitted during the formal comment period. Please send all comments to PartBLCDComments@elevancehealth.com.
