Error Description:

Home health:  The ‘From’ and ‘Through’ dates on the HH claim fall outside a HH admission period.

Avoiding/Correcting This Error

All dates of service reported on a period of care claim must be on or after the date of admission. Verify the from and through dates billed and all line-item dates of service to ensure the dates are within the admission to home health care.

*Note: Some periods of care with dates of service three years past the admission date are returning in error for U5371. The CWF will need to be updated to accommodate these later periods of care. If your claim is returning in error based on later dates of service, the claim(s) need a workaround until the CWF is updated. Contact the Provider Contact Center for resolution.
