Error Description:
Home health: The ‘From’ and ‘Through’ dates on the HH claim fall outside a HH admission period.
Avoiding/Correcting This Error
All dates of service reported on a period of care claim must be on or after the date of admission. Verify the from and through dates billed and all line-item dates of service to ensure the dates are within the admission to home health care.
*Note: Some periods of care with dates of service three years past the admission date returned in error for U537I. A correction has been installed to fix this error. Providers should resubmit any claims that were incorrectly RTP'd for the U537I reason code
