Each UTN is Valid for One-Time Use
The National Government Services Medical Review Department initiated prior authorization reviews for the following outpatient department procedures: Panniculectomy, Rhinoplasty, Vein Ablation, Blepharoplasty and Botulinum Toxin.
With each PAR that is submitted, a UTN will be issued and it is valid only for one time use on one claim submission of the HCPCS that are requested on the PAR for a period of 120 days. Any additional claims that will be submitted with similar HCPCS will require additional PAR and new UTN will have to be obtained.
Prior Authorization Reminder
A UTN will be assigned for each PAR. The PAR decisions and UTN are valid for 120 days.
