National Government Services:
Raising Awareness about Care Management

According to the CDC, six in ten adults in the U.S. have a chronic disease and four in ten adults have two or more.

As your Medicare Contractor for Jurisdiction 6 and Jurisdiction K, NGS is raising awareness of the benefits of care management services for Medicare beneficiaries with chronic conditions.

We have a series of webinars scheduled that offer our provider community information about available care management services, and guidance for proper billing of these services.   • Services typically start with a 50-minute face-to-face visit that includes a detailed history and patient exam. Use information you gather from the exam to create a written care plan.
