Oxygen CMN: Completing the Length of Need

The CMN for oxygen (CMS-484) requires the physician to indicate a LON in Section B. If a LON other than 99 (indicating lifetime) is specified, the certification will expire when the specified LON time period elapses.

It is important to remember that the LON specified in Section B is a cumulative total. For example, if the physician indicates 12 as the LON on the initial CMN for a group 1 beneficiary and indicates 24 for the LON at the time of recertification, this effectively extends the LON for an additional 12 months.

However, if the physician initially indicates a LON of 12 months for a group one beneficiary, and upon recertification the physician indicates 12 in the LON, no further payment can be issued as this would not extend the LON.

A revised CMN is required when the length of need expires. However, when the indications for a revised CMN (i.e., the LON expires) are met at the same time that a recertification CMN is due, file the CMN as a recertification CMN.
