  2. Go to PECOS to complete, review and submit the enrollment application.
  3. Sign and date the two-page certification statement in PECOS and electronically submit with your application (preferred method) or, print, sign and date the certification statement and mail to us.

Why PECOS? PECOS stands for Provider Enrollment, Chain and Ownership System web application. Each year, providers submit thousands of paper CMS-855 applications for one or more reasons, including establishing a practice, making a change to an existing practice, or joining a group practice. must process the applications received from health care providers, physicians, nonphysician practitioners and suppliers for enrollment in the Medicare Program; and, whenever an application or change to existing provider-file information occurs, National Government Services must be notified to ensure you are reimbursed the proper Medicare payment for services furnished.
