Please enter the date of service the patient last received podiatry care in MM/DD/YY or MM/DD/YYYY format and select Calculate. The date returned in the box below the horizontal rule is the next eligible date that the patient is entitled to receive routine foot care.

Note: Routine foot care services are considered medically necessary once (1) in 60 days. More frequent services will be considered not medically necessary. There must be 60 full days between the dates of service for routine foot care coverage.
