Beneficiary Name to Number Converter

This tool will help you to convert the beneficiary's name into a numeric format for easy input into the National Government Services interactive voice response (IVR) system.

In the Name = field, enter the beneficiary's name using the following format: Last Name (including titles, e.g., Jr., Sr., and III) (space) First Initial, then select the Convert button. The tool will convert the name into the numbers required by the IVR system.

Note: The IVR will only accept the entire last name and then the first initial of the first name when the name is entered — it must be entered in that order.

Enter the numeric sequence returned below in the Phone Value = field into the IVR exactly as it appears (including the #).

PTAN, Beneficiary Medicare Number and Document Control Number Converter

This tool will convert your Provider Transaction Access Number (PTAN), the beneficiary's Medicare Health Insurance Claim number (HICN), or the Medicare Beneficiary Identifier (MBI), and/or the Document Control Number (DCN) into a numeric format for easy input into the IVR system.

In the PTAN/HICN/MBI/DCN = field, enter the PTAN, beneficiary HICN/MBI or DCN and select the Convert button. The tool will convert the data entered to the numbers/characters required by the IVR system.

Note: When an asterisk "*" character is displayed — that character must be keyed into the IVR.

Enter the numeric sequence returned below in the Phone Value = field into the IVR exactly as it appears (including the #).

Reminder: You can also key in the beneficiary's HICN/MBI by keying the numbers and using the information in the IVR conversion tables below in place of the letter(s), if any. (Example: 123456789a = 123456789*21)
