Change Request 5921, dated May 7, 2008. Policy was updated
for certification and recertification requirements per
CR 5921. The Centers for Medicare & Medicaid Services
(CMS) language throughout the policy was also updated
to reflect changes in CR 5921. Although the revision
effective date is June 13, 2008, these changes became
effective on or after January 1, 2008. No change was
made to the supplemental instructions article associated
