* IVR hours represent general availability for access to all menu options. The IVR is available 24-hours a day, seven days a week. Menu options that require system access (e.g., the Common Working File) are limited to that systems availability.

Duplicate Remittance Request Form

Download and complete the Remittance Advice Request Form (Currently the form does not apply to Connecticut, Delaware, New York, and Massachusetts (00308) providers) for each duplicate remittance advice that you are requesting be printed and mailed. Once completed, send the completed form(s) along with your check or money order to the address included on the form.
